The Health Form must be filled out annually at the beginning of the program year, not at your Trailman's renewal date. All 4 pages must be filled out (with the exception of the immunizations and waiver of immunizations, those are optional). We also must have a copy of your health insurance card for the file.

Health forms are required for Trailmen to attend meetings / outings or parents to attend outings. The health form is pdf editable. You can save the file and then re-print it each following year. Just make sure the information is up to date (including changes in insurance, your Trailman's age and the date you sign the form). When you have it filled out at the beginning of the program year, bring it in to give to our Health & Safety Officer or to your Trailman's direct contact leader.
